SSN, S1, private cover — the Spanish healthcare system
Overview — Spain's public healthcare system
Spain has a universal public healthcare system (Sistema Nacional de Salud, SNS) that covers most medical costs. The system is regionally delegated — each autonomous community administers its own services. Quality varies somewhat by region (Catalonia, Madrid, Basque Country score highly; some smaller autonomous communities have longer waits).
For UK movers, three pathways: SSN registration after qualifying residence; S1 form transferring NHS entitlement for state pensioners; private cover for those not yet eligible.
SSN registration — the standard route
After establishing residencia and padrón, you can register with your local Centro de Salud and obtain a tarjeta sanitaria (health card). The registration requires NIE, residencia/TIE, padrón certificate. Process takes 2-6 weeks.
Once registered, you have access to the SSN for médico de cabecera (GP) consultations, prescriptions, hospital care, specialist appointments. Modest copays apply for some prescriptions and specialist visits depending on income.
S1 form — for UK state pensioners
UK state pensioners moving to Spain can transfer NHS entitlement to Spanish SSN via the S1 form, administered through the UK NHSBSA Overseas Healthcare Services. The S1 effectively means the UK government pays Spain for your healthcare.
S1 holders register at the local INSS office and the Centro de Salud showing the S1 form; faster than SSN for non-pensioners.
Private health insurance
Spanish private health insurance (seguro médico privado) covers private specialist consultations, premium hospital rooms, dental, optical, treatments not fully covered by SSN. Many Spanish residents have one as a top-up.
For UK movers, private cover is essential during the gap before SSN registration completes. Spanish insurers (Sanitas, Adeslas, DKV, Asisa) offer expat-suitable policies; international providers (Bupa, Cigna, Allianz Care) also viable.
The gap period
For UK movers without S1, there is typically a 2-6 month gap between arrival and SSN registration completing. Plan €50-200 per person per month for private cover during the gap.
